ABSTRACT

OBJECTIVES@#To review the effectiveness of secondary alveolar bone grafting using iliac cancellous bone in patients with unilateral complete alveolar cleft and to investigate the factors influencing it.@*METHODS@#A retrospective study of 160 patients with unilateral complete alveolar clefts who underwent iliac cancellous bone graft repair at the Department of Cleft Lip and Palate Surgery, West China Hospital of Stomatology, Sichuan University, was conducted. Eighty patients in the young age group (6-12 years) and 80 in the old age group (≥13 years) were included. Bone bridge formation was determined using Mimics software, and the volume was measured to calculate the iliac implantation rate, residual bone filling rate, and resorption rate. The factors that affected bone grafting in both subgroups were investigated.@*RESULTS@#Using bone bridge formation as the clinical success criterion, the success rate for the entire population was 71.25%, with a significant difference of 78.75% and 63.75% for the young and old age groups, respectively (P=0.036). The gap volume in the latter was significantly larger than that in the former (P<0.001). The factors that influenced bone grafting in the young group were the palatal bone wall (P=0.006) and history of cleft palate surgery (P=0.012), but only the palatal bone wall affected the outcome in the old age group (P=0.036).@*CONCLUSIONS@#The results of alveolar bone grafting for the old age group were worse than those for the young age group. The palatal bone wall was an important factor that affected alveolar bone grafting, and alveolar bone grafting in the young patients was influenced by the history of cleft palate surgery.

ABSTRACT

Introduction: Knowledge on the mating behavior and habitat requirements of large sharks, including the lemon shark, Negaprion brevirostris, is scarce, hampering conservation efforts. Objective: To describe an area in the Equatorial Atlantic used as a mating ground by N. brevirostris, as well as part of the species pre-copulatory behaviors based on citizen reports. Methods: Between 2004 and 2019, recreational divers, dive guides and rangers from the Marine Protected Area (MPA) in Fernando de Noronha Archipelago (FEN) recorded courtship behaviour, females with mating scars, pregnant females, and adult males. Results: N. brevirostris was recorded mating in shallow waters (0.5-2 m deep) during the austral summer (December to March). A specific location in the MPA, Buraco da Raquel lagoon, was the main aggregation and mating site for adult N. brevirostris in FEN. Conclusions: Citizen science records allowed the identification of shallow waters as key sites for the reproduction of this shark in FEN. Results highlight the potential of citizen science contributions to knowledge of sharks in nature and show MPAs as essential for habitat conservation of sharks with decreasing populations along the Brazilian coast, such as N. brevirostris. We present management recommendations to protect N. brevirostris there and elsewhere.


Introducción: El conocimiento sobre el comportamiento reproductivo y requisitos de hábitat de apareamiento de los tiburones grandes, incluido el tiburón limón, Negaprion brevirostris, es escaso, lo que dificulta los esfuerzos de conservación. Objetivo: Describir un área en el Océano Atlántico ecuatorial utilizada como zona de apareamiento y las interacciones precopulatorias del tiburón limón con base en informes proporcionados por ciencia ciudadana. Métodos: Buzos recreativos, guías de buceo y guardaparques del Área Marina Protegida (AMP) en el archipiélago Fernando de Noronha (FEN) llevaron registros de comportamientos de cortejo, hembras con cicatrices de apareamiento, hembras preñadas y machos adultos, entre 2004 y 2019. Resultados: Las aguas poco profundas entre 0.5 y 2 m de profundidad son utilizadas como áreas de apareamiento por N. brevirostris durante el verano austral (diciembre a marzo). Un lugar específico, dentro del AMP (laguna de Buraco da Raquel), fue identificado como el principal sitio de agregación y apareamiento de adultos de N. brevirostris en FEN. Conclusiones: Registros de ciencia ciudadana permitieron identificar aguas poco profundas como sitios clave para la reproducción de este tiburón en FEN. Los resultados resaltan el potencial de las contribuciones de la ciencia ciudadana al conocimiento de los tiburones en la naturaleza y muestran que las AMP son esenciales para la conservación del hábitat de tiburones con poblaciones decrecientes a lo largo de la costa brasileña, como N. brevirostris. Presentamos recomendaciones de gestión para proteger a N. Brevirostri, allí y en otros lugares.

Dissociative trance and possession disorder is common in clinical settings and is dif?cult to differentiate from psychotic disorder by clinicians. It has also been associated with non-suicidal self-injurious behaviours. A case of 24-year-old married female who is 4 months ANC was diagnosed with a case of trance and possession disorder with a bad obstetric history. She received treatment on OPD basis.

Abstract Background: Infantile hemangiomas (IH) occur in approximately 4% to 10% of the pediatric population. The identification of clinical subtypes and conditions that indicate increased risk for complications is essential for therapeutic success. Objectives: To identify risk factors for complications, recurrence and unaesthetic sequelae. Methods: Retrospective cohort of patients with infantile hemangiomas undergoing follow-up at the Dermatology Service of Universidade Federal de Ciências da Saúde de Porto Alegre, between 2006 and 2018. Results: 190 patients were included; 24% had some type of complication, ulceration being the most frequent, and 86% required treatment. On correlation, ulceration was statistically related to mixed IH (p = 0.004), segmental IH (p < 0.01) and location in the gluteal region (p = 0.001). The mean time of treatment with propranolol was 12.7 months. Patients with PHACES syndrome and segmental infantile hemangioma required longer treatment (p < 0.001 and p = 0.0407, respectively), as well as those who started treatment after five months of life (p < 0.0001). Recurrence occurred in 16.6% of the treated patients, all-female; 94% were located on the head and neck (mainly on the upper eyelid, cyrano, S3 segment, and with parotid involvement); 61% and 38.8% were of the mixed and deep subtypes, respectively. Approximately 1/3 of the patients had some unaesthetic sequelae. Study limitations: As this is a retrospective study, data and photos of some patients were lost. Conclusions: Mixed and segmental hemangiomas are risk factors for ulceration and sequelae. Recurrence occurs more often in females and segmental hemangiomas. Segmental infantile hemangioma and PHACES syndrome require a longer time of treatment. Specific protocols are required for infantile hemangiomas with a high risk of recurrence.

Resumen INTRODUCCIÓN: Las úlceras de Lipschütz son una causa infrecuente de úlcera genital, de alivio espontáneo y casi siempre benignas, aunque en algunas pacientes pueden dejar cicatrices genitales. Lo común es que aparezcan en mujeres jóvenes, antes del inicio de la vida sexual activa, en coincidencia con un cuadro catarral o pseudogripal. CASO CLÍNICO: Paciente de 18 años con una úlcera vulvar dolorosa, coincidente con un episodio de amigdalitis aguda y fiebre de 38 ºC. La paciente negó haber tenido relaciones sexuales. La úlcera alcanzó 3 a 4 cm, profunda, purulenta, con apertura del labio menor derecho y con importante componente necrótico. Para el control del dolor se le indicaron: corticoides, doxiciclina oral, crema con lidocaína y antiinflamatorios. Los análisis de laboratorio descartaron que se tratara de infección de trasmisión sexual. Al término del esquema terapéutico prescrito la evolución fue favorable, con desaparición de los síntomas, pero con una secuela: apertura del labio mayor derecho. CONCLUSIONES: La úlcera de Lipschütz es una causa infrecuente de úlcera vulvar. Su tratamiento consiste en el control de los síntomas y casi siempre se cura en el transcurso de 4 a 6 semanas, sin dejar lesiones. El diagnóstico solo puede establecerse luego de excluir otras causas más frecuentes de úlcera vulvar.


Abstract INTRODUCTION: Lipschütz ulcers are an infrequent cause of genital ulcer, of spontaneous relief and almost always benign, although in some patients they may leave genital scars. They usually appear in young women, before the onset of active sexual life, coinciding with a catarrhal or flu-like condition. CLINICAL CASE: 18-year-old female patient with a painful vulvar ulcer, coinciding with an episode of acute tonsillitis and fever of 38 ºC. The patient denied having had sexual intercourse. The ulcer was 3 to 4 cm, deep, purulent, with opening of the right labium minora and with a significant necrotic component. For pain control she was prescribed corticosteroids, oral doxycycline, lidocaine cream and anti-inflammatory drugs. Laboratory tests ruled out sexually transmitted infection. At the end of the prescribed therapeutic scheme the evolution was favorable, with disappearance of symptoms, but with a sequel: opening of the right labium majus. CONCLUSIONS: Lipschütz ulcer is a rare cause of vulvar ulcer. Its treatment consists of symptom control and it almost always heals within 4 to 6 weeks, leaving no lesions. The diagnosis can only be established after other more frequent causes of vulvar ulceration have been excluded.

ABSTRACT

Objective: To study the anti-hypertrophic scar effect of the six-herb Chinese medicine composition (SCMC) ointment on the rabbit ear hypertrophic scar models. Methods: The optimal formulation of SCMC ointment matrix was screened by the orthogonal designs and a series of evaluation tests. The SCMC ointment was prepared through emulsifying method. The rabbit ear hypertrophic scar models were established and used to investigate the anti-hypertrophic scar effect of SCMC ointment. Results: Our results demonstrated that all the quality control indications of the SCMC ointment met the requirements. Anti-hypertrophic scar activity results showed that all the rabbit ear scar tissues appeared different degrees of shrink and fading, and took an unobvious but palpable shift from hard to soft texture with the low, middle and high concentration SCMC ointments treatments in vivo. Additionally, on 21st day the scar area and thickness in different concentrations of SCMC ointment groups were significantly reduced than control group, in a concentration-dependent manner. The immunohistochemical results also indicated that the SCMC ointment had good anti-hypertrophic scar properties and could inhibit hypertrophic scar formation. Conclusion: The SCMC ointment could improve the blood circulation condition of hypertrophic scar tissues. Our research has demonstrated the Chinese medicine composition ointment with good anti-hypertrophic scar properties that could be used to treat hypertrophic scars. Meanwhile, it provides a theoretical basis for further clinical application.

ABSTRACT

Resumen: La infección del tracto urinario (ITU) es una de las infecciones bacterianas mas frecuentes en la edad pediátrica, pero su diagnóstico y manejo se pueden ver complicados por lo inespecífico de sus sín tomas y signos de presentación, la dificultad en la interpretación de los exámenes, especialmente en niños mas pequeños, y por un pronóstico respecto a daño renal muchas veces incierto. En los últimos años, se ha evidenciado una modificación significativa en el enfoque diagnostico y terapéutico de esta patología, surgiendo la necesidad de actualizar las recomendaciones previas. El propósito de esta re vision es contribuir a reducir la variabilidad de la práctica clínica en el manejo de ITU en la población pediátrica, mejorando la detección y manejo de la patología estructural y otros factores de riesgo de daño renal, evitando acciones innecesarias en aquellos niños con bajo riesgo. En esta primera parte, se presentan las recomendaciones en cuanto a diagnóstico y manejo de la ITU en pediatría. En la segunda parte se detalla su estudio, prevención y seguimiento.


Abstract: Urinary tract infection (UTI) is one of the most frequent bacterial infection in pediatrics. However, its diagnosis and management can be complicated due to the nonspecific clinical presentation, the difficulty of exams interpretation, especially in younger children, and an uncertain prognosis regar ding renal damage. In recent years, significant worldwide change has come in treatment, diagnosis, and images studies, we have decided to update the current recommendations on UTI management published by the Pediatric Nephrology branch of Chilean Pediatrics Society in previous years. The purpose of these recommendations is to reduce the variability of clinical practice in management of UTI in our pediatric population, favoring diagnostic and therapeutic interventions in the most ap propriate way, improving detection and management of structural pathology and other risk factors of renal damage, avoiding unnecessary actions in children with low risk. This first part includes diag nosis and treatment recommendations of urinary tract infection in pediatric age. In the second part the study, prevention and monitoring of urinary tract in pediatric age is detailed.

Introducción: La cicatriz queloide forma parte de las cicatrices patológicas por exceso; es una afección que se caracteriza por el depósito excesivo de colágeno en la dermis y tejido celular subcutáneo. El proceso por el cual se desarrolla no es del todo conocido, es más frecuente en la raza negra y afecta en igual proporción a hombres que mujeres. Objetivo: Informar a la comunidad médica sobre experiencia en el manejo y tratamiento con dos pacientes portadores de lesiones queloides de gran tamaño. Caso clínico: Se presentan dos pacientes con cicatriz queloide gigante a partir de ambos lóbulos auriculares y cuello anterior; se expone la conducta terapéutica basada en la combinación de tres modalidades de tratamiento: cirugía; infiltración con acetónido de triamcinolona, 1 ml (40 mg) desde el día del acto quirúrgico, se repitió cada 15 días; junto a presoterapia con crema esteroidea. Conclusiones: Aunque no existe un tratamiento ideal, se debe tener en cuenta al tratar esta enfermedad si se está ante una lesión recidivante, el tiempo de aparición de la tumoración, sus características clínicas; de seleccionarse el tratamiento quirúrgico como opción terapéutica, debe acompañarse de otras modalidades de tratamiento(AU)


Introduction: The keloid scar is part of the pathological scars by excess, is a condition characterized by the excessive deposit of collagen in the dermis and subcutaneous cell tissue, the process by which it develops is not entirely known, it is more frequent in the black race, affects in equal proportion men than women. Objective: To inform the medical community of the experience in handling and treating two patients with large keloid lesions. Clinical case: Two patients with giant keloid scars are presented from both earlobes and anterior neck where therapeutic behavior based on the combination of three treatment modalities is exposed: surgery, triamcinolone acetonide 1 ml (40mg) infiltration beginning on the first day after surgery, repeated every 15 days and pressotherapy with steroid cream. Conclusions: Although there is no ideal treatment, it should be taken into account when treating this disease if we are facing a recurrent injury, the time of onset of the tumor, its clinical characteristics, surgical treatment should be selected as a therapeutic option, accompanied by other forms of treatment(AU)

OBJECTIVE@#To observe and compare the clinical efficacy of CO₂ fractional laser in the early control of scar post-secondary repair in patients with a cleft lip.@*METHODS@#In the treatment group, 43 patients with secondary repair of cleft lip were treated via CO₂ fractional laser. The control group covered 70 patients post-cheiloplasty. The effect in the two groups after six months was compared. The duration from the beginning of the laser treatment to surgery and patient gender were analyzed to determine if they affected the efficacy of the laser treatment for scars.@*RESULTS@#1) The curative effect in the treatment group was better than that in the control group (P<0.000 1), the total effectiveness rate was 90.7% in the treatment group. 2) No significant statistical difference in efficacy was observed between men and women (P=0.487). Moreover, no significant statistical difference in efficacy (P=0.055) was observed among patients one year after surgery, within the duration of <3 months, and within the duration of ≥3 months.@*CONCLUSIONS@#CO₂ fractional laser had a definite effect on the treatment of scar post-secondary repair in patients with a cleft lip. No significant correlation was observed between treatment effect and patient gender, and no difference was noted among patients one year after surgery. In the beginning of the laser treatment, no difference was observed between the durations of <3 months and ≥3 months after the reconstruction, Therefore, early intervention of scars a year after secondary repair of a cleft lip can achieve good results.

ABSTRACT

Background: The visibility, vulnerability and social stigmata of facial scars whether by burn, nevi or trauma can be compelling for the patient as well as challenging for the surgeon. Restoration to normal form and aesthetics require tissue replacement which has good colour and texture match and produce minimal visible scarring.  Although many other options are available for a given defect, tissue expansion offers the best alternative which meets almost all the criteria of an ideal procedure.Methods: Among 92 patients with deformities over various facial subunits were operated and expanders 50 ml to 300 ml inserted subcutaneously adjacent to the scar. Prior planning, accurate measurement and choice of ideal expander is extremely important. A precise and practical method of calculation for determination of amount and duration of expander was used. Any secondary deformity to adjoining vital structures was avoided.Results: Results were meticulously and critically analyzed. Different shapes, dimensions and volume of expanders were used depending on the anatomical site which was to be expanded. A total of 118 expanders were inserted in 92 patients. The average volume of tissue expanders used was 170.33 ml. Majority of the expanders used had volume of 200 ml (62.71%). Post-expansion volume was 240.67 ml and the over expansion done was 41.3% over the pre-expansion volume of 170.33 ml. Surgical outcome and cosmesis was assessed by the patient’s perspective and was considered fair by 57.61% patients.Conclusions: The study underlines the clinical application, reasons for overexpansion as well as shortcomings and complications of tissue expansion.

ABSTRACT

La Cromoblastomicosis es una micosis subcutánea producida por hongos melanizados conocidos como dematiáceos, comúnmente secundario a un proceso traumático previo. Actual mente la incidencia de esta patología es desconocida. Se presenta el caso de un hombre de 64 años originario de la comunidad de Coyomeapan, Puebla, México, quien acude por falta de cicatrización de una úlcera en hueco poplíteo derecho generada por quemadura con candil de petróleo hace cuatro años. Refiriendo dolor con leve dificultad para la deambulación y sin evidencia de sangrado activo. Se realiza raspado de la lesión con presencia de células fumagoides posteriormente se aisló Cladophialophora carrionii en Agar Boreli. Se inició tratamiento con terbinafina para su recuperación. El conocimiento de este tipo de micosis aumenta la posibilidad de un diagnóstico temprano y certero para evitar complicaciones derivadas de la cronicidad o retraso del diagnóstico.


Chromoblastomycosis is a subcutaneous mycosis produced by pigment-producing fungi known as dematiaceous, commonly after a previous traumatic process. Currently the incidence of this pathology is unknown. We present the case of a 64-year-old man from the community of Coyomeapan, Puebla, Mexico, who came for lack of healing of an ulcer in the right popliteal socket generated by burning with an oil lamp four years ago. Referring pain with mild difficulty in ambulation and without evidence of active bleeding. Scraping of the lesion was performed with the presence of fumagoid cells. Cladophialophora carrionii was isolated on Boreli Agar. Treatment was started with terbinafina for recovery. The knowledge of this type of mycosis increases the possibility of an early and accurate diagnosis to avoid complications derived from the chronicity or delay of the diagnosis.

ABSTRACT

Objective To prepare the liposomes of salvianolic acid B modified with cell penetrating peptide TAT (SAB-TAT-LIP), of which has effects on preventing and treating hypertrophic scars (HS), and establish the method of quality evaluation, as well as preliminarily investigate the effect on the proliferation and migration of human skin fibroblasts (HSF). Methods Liposomes were prepared by pH gradient reverse-phase evaporation method, and the entrapment efficiency was measured by ultrafiltration. Box-Behnken design was performed to optimize the formulation of liposomes by using encapsulation rate as evaluating index. The physicochemical properties of liposomes including morphology, entrapment efficiency, particle size, zeta potential, in vitro release and transdermal absorption, and stability were studied. In addition, the effect of liposomes on proliferation of HSF was examined by MTT assay, and the effect of liposomes on migration of HSF was investigated by scratching method and Transwell assay. Results Based on the optimal formulation of SAB-TAT-LIP, the entrapment efficiency of salvianolic acid B was (86.70 ± 0.85)%, the average particle size was (219.90 ± 5.09) nm, and the zeta potential was (-9.25 ± 0.92). The in vitro 24 h cumulative release was 62.49% of the total drug with no burst effect. The in vitro 32 h cumulated skin penetration rate was 17.21%, the permeance rate was (28.33 ± 4.9) μg/(cm2∙h), and the retention volume of dermis was (44.39 ± 6.87) μg/cm2. The stability was good when placed at 4 ℃ for 10 d. The in vitro cell studies showed that SAB-TAT-LIP can significantly inhibit the proliferation, migration and invasion of human skin fibroblasts, compared with the control group (P < 0.01). Conclusion The optimized SAB-TAT-LIP have higher encapsulation efficiency, smaller particle size, good sustained release effect, and good dermal retention effect which all satisfy the in vitro release and transdermal regulation of local transdermal preparation, and it can significantly inhibit the proliferation, migration and invasion of human skin fibroblasts in vitro.

In mammalian retina,Müller cells are dominating macroglial cells and span the entire retina.These cells perform a variety of physiological roles to maitain the normal function of retina.However,Müller cells become ‘reactivity'in response to every pathological changes in the retina.Reactive Müller cells play an important role in retinal damage and repair.Reactive gliosis is a complex process that is considered to represent a cellular response to protect the retina from further damage and to promote its repair following pathological insult in the early stage of retina injury.Reactive Müller cells protect the tissue and preserve tissue function by releasing neurotrophic factors,and may contribute to retinal regeneration by generating neural progenitor.However,continued proliferation of Müller cells can also lead to cell dysfunction and damage of photoreceptors and neurons.What's more,Müller cell gliosis may result in the formation of glial scars,which can inhibit retinal remodeling and reprograming of the injured retina.A better understanding of the role and mechanism of Müller cells in retinopathy is essential for the efficient therapeutic strategies of retina diseases.

Objectives: To develop and assess Pediatric AppropriatenessEvaluation Protocol for India (PAEP-India) for inter-rater reliabilityand appropriateness of hospitalization.Design: Cross-sectional study.Setting: The available PAEP tools were reviewed and adaptedfor Indian context by ten experienced pediatricians followingsemi-Delphi process. These PAEP-India tools were applied to cases to assess appropriateness ofadmission and inter-rater reliability between assessors.Participants: Two sets of case records were used: (i) 274cases from five medical colleges in Delhi-NCR [?28 days (n=51);>28 days to 18 years (n=223)]; (ii) 622 infants who werehospitalized in 146 health facilities and were part of a cohort (n=30688) from two southern Indian states.Interventions: Each case-record was evaluated by twopediatricians in a blinded manner using the appropriate PAEP-India tools, and ‘admission criteria’ were categorized asappropriate, inappropriate or indeterminate.Main outcome measures: The proportion of appropriatehospitalizations and inter-rater reliability between assessors(using kappa statistic) were estimated for the cases.Results: 97.8% hospitalized cases from medical colleges werelabelled as appropriate by both reviewers with inter-rateragreement of 98.9% (k=0.66). In the southerm Indian set ofinfants, both reviewers labelled 80.5% admissions as appropriatewith inter-rater agreement of 96.1% (k= 0.89).Conclusions: PAEP-India (newborn and child) tools are simple,objective and applicable in diverse settings and highly reliable.These tools can potentially be used for deciding admissionappropriateness and hospital stay and may be evaluated later forusefulness for cost reimbursements for insurance proposes.

ABSTRACT

Background: Acute generalised exanthematous pustulosis(AGEP) is a rare, cutaneous reaction characterised bysudden onset of numerous, non-follicular, sterile pustuleson oedematous erythematous skin, accompanied by feverand neutrophilia. AGEP is predominantly drug-induced. Skinlesions appear rapidly within 1-3 days of drug exposure andupon drug withdrawal, resolve rapidly within 15 days.Objective: To determine the clinical characteristics, culpritdrugs and outcome of patients with AGEP.Methods: A retrospective note review of all AGEP patientsseen from 2001-2015.Results: Among 21 AGEP patients, 76% were Malays, 9.5%Chinese, 9.5% Indians, and 5% Iban. Sixteen were femalesand 5 were males. Median age of patients was 40 years (IQR:26). The main culprit drug was amoxicillin (10 cases),followed by cloxacillin (three cases), phenytoin (two cases)and one case each of carbamazepine, sulphasalazine,allopurinol, cephalexin, ceftriaxone, celecoxib and herbalproduct. The median time from drug initiation to onset ofAGEP was 3 days (IQR: 5.5). Fever was documented in 52.4%, mucosal involvement 9.5%, purpura 4.7% and blisters4.7%. Neutrophilia was observed in 63.6% of patients andeosinophilia in 28.5%. While most patients requiredadmission (67%), all achieved complete recovery within 15days without any sequela.

Introducción: Muchos de los traumatismos de gran impacto ocasionados por accidentes de tránsito producen fracturas expuestas en los miembros inferiores, con exposición de las partes blandas. Se presenta el caso clínico de un paciente con diagnóstico de osteomielitis crónica de tibia izquierda, post operado de colgajo fasciocutáneo de gemelo interno e injerto de piel en la pantorrilla. Descripción del caso: Varón de 27 años internado 45 días en la Unidad de Cirugía Plástica del Hospital de Clínicas, recibe 16 sesiones de tratamiento kinésico, desde la etapa pre, post quirúrgica y alta hospitalaria. En la evaluación pre quirúrgica se observa una herida abierta en la zona de la pierna izquierda cubierta por apósitos y sistema de aspiración y compactación al vacío, cicatrices adheridas en la rodilla, limitación de la movilidad del tobillo y rodilla, secuela de las lesiones e intervenciones anteriores, es sometido a una cirugía reconstructiva a nivel de la pierna. Los objetivos kinésicos planteados fueron: educar al paciente y su familia sobre los cuidados del injerto y la cicatrización, mejorar la movilidad articular, flexibilidad, fuerza muscular, disminuir los acortamientos musculares y, flexibilizar las cicatrices adheridas. Discusión: Los traumatismos de alta energía en miembros inferiores se asocian con frecuencia a defectos de partes blandas y su reconstrucción puede ser una tarea desafiante, la rehabilitación física desde el cuidado post quirúrgico inmediato hasta el alta, devolvieron al paciente su máxima independencia funcional en un corto periodo de tiempo en tras un largo periodo de hospitalización.


Introduction: Many of the high impact injuries caused by traffic accidents produce compound fractures in the lower limbs, with soft tissue exposure. We are presenting the clinical case, diagnosed with chronic osteomyelitis of the left tibia, post-operated fasciocutaneous flap of the internal calf and skin graft in the calf. Case Description: A 27-year-old male patient is admitted to the Plastic Surgery Unit of the Hospital de Clínicas for 45 days, receives 16 sessions of kinesthetic treatment, from the pre, post-surgical and hospital discharge stage. In the pre-surgical evaluation, an open wound is observed in the area of the left leg covered by dressings and aspiration system and vacuum compaction, scars attached to the knee, limitation of ankle mobility and knee sequelae of previous injuries and interventions, is undergoing reconstructive surgery at the level of the leg. The kinesic objectives proposed were: educate the patient and his family about graft care and healing, improve joint mobility, flexibility, muscular strength and reduce muscle shortening, making the attached scars more flexible. Discussion: High-energy trauma in the lower limbs is often associated with soft-tissue defects and its reconstruction can be a challenging task. Physical rehabilitation from immediate postsurgical care to discharge has restored the patient's maximum functional independence in a short period of time after a long period of hospitalization.

Introdução: O microagulhamento, também conhecido como terapia percutânea de indução de colágeno, é procedimento seguro e eficaz, usado para rejuvenescimento, melhora de cicatrizes, melasma, alopecias e outras indicações clínicas, bem como drug delivery em geral. Apesar dos benefícios e da segurança da técnica, o controle da dor ainda é seu maior fator limitante. Objetivo: Avaliar se a realização de drug delivery de anestésicos tópicos imediatamente antes do microagulhamento pode diminuir a sensação de dor. Métodos: Estudo-piloto de casos, split face com nove pacientes consecutivos, buscando tratamento para cicatrizes de acne, rejuvenescimento e melhora da firmeza da pele. Após limpeza da pele, foi aplicado creme anestésico tópico em toda a face, seguido de drug delivery deste com roller de 0,5mm somente no lado esquerdo. Imediatamente após, o anestésico foi removido de toda a face e realizado o microagulhamento nos dois lados com roller de 1mm. Resultados: O lado esquerdo da face, onde foi realizado drug delivery do anestésico tópico antes do microagulhamento, apresentou significativa diminuição da dor (p < 0,01) com média de 3,33 (± 1,49) quando comparado com o lado direito da face [média de 5,22 (± 1,74)], no qual foi aplicado o mesmo anestésico e pelo mesmo tempo. Conclusão: Neste ensaio, a técnica de drug delivery do anestésico tópico foi eficaz e segura para diminuir a sensação de dor durante o microagulhamento.


Introduction: Microneedling, also known as percutaneous collagen induction therapy, is a safe and effective procedure, mostly used for rejuvenation, treatment of scars, melasma, alopecia, and other conditions, as well as for drug delivery. Despite the safety and benefits of this technique, the control of pain during the procedure is its major limiting factor. Objectives: To assess whether the drug delivery system for topical anesthesia immediately before a microneedling session is capable of reducing the pain. Methods: A split-face pilot study was carried out with nine consecutive cases of microneedling that had been indicated for the treatment of acne scars, rejuvenation and collagen induction. All patients underwent cleansing of the skin followed by the application of topical anesthetics on the entire face, and drug delivery with a 0.5mm roller only on the left hand side of the face. Next, all anesthetics were removed and microneedling with a 1.0mm roller was performed on both sides of the face Results: There was a significant reduction of the pain on the left hand side of the face, where the drug delivery was carried out (p <0.01), with a mean value of 3.33 (± 1.49) on the Visual Analogue Scale as compared to the right hand side, which yielded a mean value of 5.22 (± 1.74). Conclusion: The topical anesthetic drug delivery technique was effective and successful in reducing the pain during microneedling procedures carried out in the study's patients group.

Introdução: As cicatrizes de acne deprimidas apresentam-se sempre como um desafio terapêutico. Lasers fracionados, abordagens cirúrgicas e peelings químicos têm tentado oferecer resultados cosméticos aceitáveis, mas continua difícil a escolha do tratamento ideal. Objetivo: Avaliar a eficácia da indução percutânea de colágeno com agulhas associada a radiofrequência pulsada com multiagulhas em cicatrizes de acne deprimidas. Métodos: Estudo retrospectivo da segurança e efetividade da técnica mediante avaliação dos resultados por aplicação de questionário de satisfação aos pacientes e julgamento clínico e fotográfico pelo dermatologista. Resultados: Foram avaliados 13 pacientes com idade entre 21 e 37 anos, submetidos à associação, 100% dos quais relataram satisfação com os resultados, enquanto na avaliação comparativa das fotografias o índice de melhora foi de 50% em dois pacientes e de 70% em seis pacientes. Conclusão: Essa nova proposta terapêutica se apresenta como alternativa ao tratamento de cicatrizes de acne deprimidas.


Introduction: Depressed acne scars always arise as a therapeutic challenge. Fractional lasers, surgical approaches and chemical peels have attempted to offer acceptable aesthetic results, however it is still difficult to determine the ideal treatment. Objective: To evaluate the efficacy of Percutaneous Collagen Induction with needles (IPCA®) associated with Pulsed Radiofrequency with Multineedles (RFPM®) in depressed acne scars. Methods: Retrospective study of the safety and effectiveness of the technique through the evaluation of results based on a satisfaction questionnaire answered by the patients and the clinical and photographic assessment carried out by a dermatologist. Results: Thirteen patients (aged between 21 and 37 years) who had undergone the combination treatment were evaluated. All (100%) reported satisfaction with the outcomes, while in the comparative evaluation of photographs yielded an improvement rate of 50% in 2 patients and of 70% in 6 patients. Conclusion: This new therapeutic proposal arises as an alternative in the treatment of depressed acne scars.

RESUMEN: La piel es el órgano más extenso en el ser humano, su integridad representa protección contra diferentes agentes químicos, biológicos y mecánicos. Las lesiones ocasionadas en este tejido se resuelven mediante la formación de una cicatriz, sin embargo, diferentes alteraciones moleculares pueden sobre estimular este proceso, lo que conlleva a la formación de cicatrices aberrantes (hipertrófica o queloide). El tratamiento más recomendado para este tipo de lesiones es la aplicación intralesional del acetónido de triamcinolona (AT) y por otro lado, la dehidroepiandrosterona (DHEA) es una pro-hormona que posee una gran variedad de efectos biológicos como: regulación de la síntesis de fibras de colágeno, protección celular, propiedades antitumorales, antiinflamatorias y antioxidante. En este trabajo, se estudió la combinación de AT-DHEA sobre la proliferación y muerte celular en la línea celular de fibroblastos 3T3-L1. Los resultados mostraron que la AT a 100 M y la DHEA a 1000 M inhiben la proliferación en un 50 y 40% respectivamente. La combinación de AT-DHEA (10000-10 M) inhibe la proliferación celular e inducen muerte celular programada, entonces esta combinación pudiera utilizarse en cicatrices hipertróficas o queloides para su eliminación.


ABSTRACT: The skin in the human is the largest organ, his integrity represents protection against various chemical, biological and mechanical agents. The injuries in this tissue are solved by forming a scar, however, different molecular alterations may overstimulate this process, leading to the formation of aberrant scars (hypertrophic or keloid). The most recommended treatment for such injuries is the intralesional application of triamcinolone acetonide (TA) and on the other hand, dehydroepiandrosterone (DHEA) is a pro-hormone that has a wide variety of biological effects such as regulation of the synthesis of collagen fibers, cell protection, anti-tumor properties, anti-inflammatory and antioxidant. In this paper, the combination of AT-DHEA on proliferation and cell death in fibroblast cell line 3T3-L1 was studied. The results showed that the AT 100 and 1000 M DHEA to inhibit proliferation by 50 and 40% respectively. The combination of AT-DHEA (10000-10 M) inhibits cell proliferation and induce programmed cell death, so this combination could be used in hypertrophic or keloid scars for disposal.

ABSTRACT

PURPOSE: Reconstruction of severe postburn hand deformities with flexion or extension contractures with finger webbing deformities, large hypertrophic scars of dorsal hand are frequently encountered problems in burn hand surgery. To obtain the good results after correction of various type of postburn scar hand deformities, we have used the sophisticated reconstructive procedures such as scar contracture release, skin graft, and use of acellular dermal matrix (ADM). We report reliability and usefulness of these novel updated procedures according the type of postburn hand deformities, and reviewed the literatures. METHODS: We had 82 postburn hand deformities. Among them we selected 7 patients of severe postburn hand deformities, which had different affected sites involving over 1/3 of hand. To reconstruct the finger flexion contractures, the scar contracture release and full thickness skin graft was most frequently performed. For correction of finger webbing deformities, the 5 flap Z-plasty for 1(st) web, dorsal and volar interposition flap for 2, 3 and 4 web, FTSG were used. The diffuse hypertrophic scar of dorsum of hand was reconstructed with total excision of scars, skin coverage with one piece of medium thickness STSG, and postoperative clenched hand position. The postburn palmar contractures was reconstructed with extensive contracture release followed by resurfacing with ADM (AlloDerm™) and thin STSG. The severe postburn abduction contractures of wrist was treated by total excision of scars, ADM (CGDerm™), and thin STSG. RESULTS: After 1 month to 1.6 years follow up, relatively satisfactory results were obtained in all patients. As complications, 1 case of recurrent palmar contractures, which was reconstructed with ADM (AlloDerm™) with thin STSG, were noticed. CONCLUSION: The postburn finger flexion contractures could be managed by the scar contractures release and FTSG. This method is very safe and reliable. For reconstruction of postburn finger webbing deformities, it is mandatory to use 5-flap Z-plasty for 1(st) webbing deformities, and dorsal and volar interposition flap for 2, 3 and 4(th) webbing deformities concomitantly with resurfacing with FTSG. The diffuse hypertrophic scars of dorsum of hand was managed by total excision of scars, resurfacing with one large piece of over medium thickness STSG, and postoperative clenched hand position. After release of scar contractures of hand, acellular dermal matrix (ADM) with thin STSG can be used in case of deficient FTSG donor site.
